Clearances

All athletes must complete 5 requirements in order to be cleared to compete:

• Fill out both Physical forms 
• Fill out the Code of Ethics Form
• Sign the Concussion Form/Consent for our trainer to train you 
• Fill out an Emergency Card (Pick up in ASB office)
• Purchase of ASB Card (Purchase card in ASB office- $45)

Turn in all forms to ASB
